Taxi/Limousine Drivers Application

NOTE: THE CITY OF WOODSTOCK DOES NOT PRO-RATE LICENCE FEES

The following needs to be attached to the application:

A valid "G" drivers license issues by the Province of Ontario, which is in good standing according to the records of the Ministry of Transportation. A copy will need to be taken at City Hall.

I authorize the City of Woodstock to obtain my drivers’ record information from the Ministry of Transportation’s Authorized Requester Information Services for the purposes of this Taxi/Limousine Driver’s Licence
The licensee hereby certifies to:

1. being eighteen (18) years of age or older;
2. being a citizen of Canada or a landed immigrant or produce a valid work permit issued by the Canadian Government;
3. advise the City of any changes in their driver’s licence status;
   
   
4. advise the Corporation of any medical or eye problems which could prevent or impede their ability to safely operate a vehicle;
5. accurately complete and file the driver daily trip records;
6. charge fares which are consistent with the provisions of this Chapter;
7. advise the Clerk or their designate of any address change;
8. be civil and behave courteously when driving a cab or limousine licensed under this Chapter;
9. be properly dressed, well groomed, neat, and clean in appearance when driving a cab or limousine licensed under this Chapter;
10. serve the first person requiring the service of the vehicle;
11. have no more than nine (9) demerit points recorded on their driver’s licence in the previous twenty-four (24) months;
12. drive the taxi/limousine directly to the destination unless the passenger authorizes the driver to do otherwise and not to pick up passengers from another location unless specifically authorized to do so by the passengers in the vehicle;
13. display the driver’s licence issued under the provisions of this Chapter in a place visible to the passengers when conveying passengers;
14. supply a signed receipt to any passenger requesting same;
15. convey only the number of passengers consistent with the number of seat belts in the vehicle;
16. limit their driving time period to not more than twelve (12) consecutive hours in a twenty-four (24) hour period, adhere to the provisions of the Smoke Free Ontario Act and the City of Woodstock Smoke Free Workplaces and Public Places By-law;
17. having not been convicted of an indictable criminal offence for which they have been sentenced to imprisonment for three months or more in the past five (5) years; or

• having not been convicted of any sexual offence for which they have been sentenced to imprisonment for three months or more; or
• ii. having not been charged with any sexual offence that is currently before the courts; or
• iii. having not been convicted of any two of the following in the past five (5) years:
                  • impaired driving;
                  • dangerous driving;
                  • driving with over 80 mgs of alcohol in 100ml of blood; or
                  • criminal negligence involving the use of a motor vehicle.

I hereby certify that I have read and understand the information contained in this application. I further certify that the information contained in this application is true and I agree that I will abide by all of the provisions of the City of Woodstock Municipal Code Section 643 and amendments thereto and any other applicable municipal regulations that
may apply to the operation of a Taxi/Limousine Driver’s licence. The licence fee has been submitted with this application.

This licence is granted for one year from February 15th in the current year and expires on the 15th of February one year after issue or sooner if the licence is revoked.

Issuance of this licence will be subject to the following department approvals:
